
Create Your Own Web Form
- Arts and Sciences Technology Services offers free, self-service web form hosting.
- Web forms are handy for collecting information from a group of people. They are just like paper forms but are online. Examples of web forms include RSVPs, registrations, and informal surveys.
- At any time, you can view and download the data you have collected in spreadsheet form, in case you want to do a mail merge or manipulate the results.
Who Can Create a Form
- You must be an Arts and Sciences faculty or staff member.
- Our web forms do not require an account for a person to complete the form. As such, do not include or request any sensitive or FERPA-protected information.
How to Request a Form
- Complete this web form to request your web form.
- You will receive your account and password via email so that you can login and design your form.
